How Can We Help You?
When a client makes an enquiry we take contact details, purpose of the contact and the needs to be fulfilled. We then arrange to complete a full assessment to be completed at the client’s place of residence or hospital. Following the assessment a care plan will be devised which our professional care team will follow. During this process we will also provide information and support with the following, if applicable:
- Respite hours for family caregivers
- Power of Attorney assignment
- Estate Planning
- Benefits; Clifton Homecare have a supply of Attendance Allowance forms & we will help you complete these if you so wish
- Senior Social Events
- Blue Badge Application
- Community Based Programs
- Transport Services
- Coordinating Doctor’s Appointments
