When a client makes an enquiry we take contact details, purpose of the contact and the needs to be fulfilled. We then arrange to complete a full assessment to be completed at the client’s place of residence or hospital. Following the assessment a care plan will be devised which our professional care team will follow. During this process we will also provide information and support with the following, if applicable:

  • Respite hours for family caregivers
  • Power of Attorney assignment
  • Estate Planning
  • Benefits; Clifton Homecare have a supply of Attendance Allowance forms & we will help you complete these if you so wish
  • Senior Social Events
  • Blue Badge Application
  • Community Based Programs
  • Transport Services
  • Coordinating Doctor’s Appointments

Navigating through the myriad of care services available in the community can often be cumbersome and confusing. For help in identifying the best care service options for you or your loved one, Clifton Homecare can help ease the burden. We will work with the attending Physician, Discharge Planner, family and the client to determine the care plan and agree on the specific goals to maximise the healing process.

Other Services

Clifton Homecare will also assist in coordinating and referring other services needed such as:

  • Medical Equipment/Aids
  • Medication Management
  • Home Modifications
  • Medical Alert System
  • Community Based Referrals and Programs, such as direct referral to the falls team
  • Coordinating Health care and specialist appointments

Our team can help with Benefit Determination. We can help to identify any programs, community resources, or benefits that might be available for clients. Alternatively, your local authority will be able to provide you with free financial support & advice. You can telephone Lancashire County Council on 0300 1236720.

Clifton Homecare will support you to access NHS funding if you are eligible. NHS; Continuing Healthcare is accessed via your District Nursing team.

Clifton Homecare will also provide assistance where possible, with completing Attendance Allowance or Blue Badge forms. Attendance Allowance is a non means tested benefit which helps to pay towards care costs.
