Care and Support

Care and Support

Our Care and Support Services are personal and tailored to you, enabling you to live as fulfilled and independently at home as possible.

Personal Care:
Our team of carers are experienced in care delivery and trained to a very high standard. We will endeavour to provide you with a carer you feel comfortable with as we realise to get the most out of any relationship a good rapport is essential.

We are passionate about ‘getting it right’ and clients and family being happy with the level of service provided. To ensure this, Care Plans and Risk Assessments are discussed with clients and families and reviewed regularly.

In order to provide excellent care we will need to work alongside health care professionals such as Doctors, District Nurses, Community Matrons, Occupational Therapists, Physiotherapists, Dietitians, Continence Advisors or Specialists. Our experience and desire to personalise care packages is essential for us to fulfil this role.

Meal Preparation:
Overall health and well-being can be improved by ensuring your loved one is receiving healthy, well-balanced meals. In addition to tasting better, nutritious meals play an important role in maintaining good health.

A Clifton Homecare caregiver can perform everything from grocery shopping to preparing meals in accordance with any special diet, to serving the meal, providing eating assistance and cleaning up afterwards. Just let us know your situation and we will provide tailored services to your individual needs.

Grocery Shopping:
Your caregiver can provide you or a loved one with assistance in grocery shopping, picking up prescriptions, organising incoming mail and going to the post office, and any routine personal errands you or a loved one might require.

Light Housekeeping:
Your caregiver can help you or a loved one with services such as vacuuming, washing dishes, laundry, dusting, bed making, sweeping, mopping floors, taking out the rubbish, and any other light housekeeping items you or a loved one might require. Let us know your situation and we will provide tailored services to your individual needs.

Companionship Care

Companionship Care

We are more than happy to escort you or a loved one on outings to places such as the theatre, cinema, for a meal or out shopping. One of the main aims for domiciliary care is to support clients with the opportunity to continue to live in their own home.

Clifton Homecare is different to other home care providers though, as we realise the importance of stimulation and entertainment. Age, physical and mental health and disability does not reduce the need or desire for activities and hobbies. We aim to aid you or your loved one to live life to the full, with enjoyment and pleasure. If going out of the home environment is impossible then we are happy for example, to spend time chatting, reading or making crafts.

At Home Outside the Home
Conversation and Company
Many people we support simply want conversation and company. Sharing stories, talking and laughing all help to bring wellness and enjoyment in the home.
Trips and Outings
Our regular visits to local garden centres, attractions, tea dances, cafes and restaurants are always a hit, especially the annual Christmas lunch!
Hobbies and Interests
Whatever your passion is we can help with hobbies such as sewing, knitting, gardening, reading or baking.
Escorting to Appointments
We’re more than happy to accompany you to the doctors, dentists or even the shops!
Crafts and Creating
Whether you enjoy painting or completing jigsaws we can help with your creation.  We also hold Christmas craft activity sessions at our offices which always prove popular!
Family Sessions and Social Groups
We regularly host informative client and family sessions and social groups at our offices and can even help you get there.

Dementia Care

Dementia Care

We aspire to support people living with Dementia to live healthy, active and fulfilling lives promoting choice, dignity and well-being. Our local care management team coordinate experienced and specialised care staff to provide one to one Dementia services at home.

Our Dementia home care service is tailored to meet your individual need starting from as little as one hour to 24 hours a day, 7 days a week including waking night care.

Clifton Homecare’s Dementia care service is led by a SPECAL Practitioner trained by the Contented Dementia Trust at their Centre of Excellence at Burford. Our SPECAL Practitioner is trained to understand the disability of Dementia from the perspective of the person with the condition and to deliver coaching to all those who come into contact with them so that everyone is on side and on message to deliver a therapeutic outcome for all concerned.

All staff receive a basic induction in the SPECAL method. The SPECAL method is a positive way of managing Dementia, rooted in person centred care to promote well-being 24 hours a day, every day for life. The method uses the analogy of a photograph album to describe, in a particular way, how normal memory works, the impact of ageing and the single dramatic change that occurs with the onset of Dementia.

All of our staff are trained to use the SPECAL Photograph Album as part of an integrated communication toolkit and to follow SPECAL’S Three Golden Rules to sustain well-being:-

  • Don’t ask direct questions
  • Listen to the expert – the person with Dementia
  • Don’t contradict

Clifton Homecare works closely with the network of family and friends, valuing each person’s role and helping them to promote a sustainable sense of well-being not only for the person with Dementia but also for everyone else involved.

Live in Care

Live in Care

Given the choice, many of us would rather continue living at home whilst receiving support but tend to assume care homes are the best or only option.

Live in care enables you to get the professional support you need without leaving the comfort of home and helps you maintain the independence, choice and dignity that are vital to your health and happiness.

There’s none of the upheaval, stress or uncertainty that comes with moving home and your routine can stay the same. You can continue to lead your own life in familiar surroundings, with the support of our professional carers trained to provide you with exceptional and compassionate care.

Our live in care service is flexible around your needs. You decide how many hours in the 24-hour window you will need practical assistance to be provided and a carer will be hand-picked for you to become your 24 hour home care support. They will be your companion and reside in your home, day and night, helping you to carry out those tasks you need them to and more importantly, being a familiar face and a source of company.
